Application Process

When the time comes to consider long-term care contacting the local Community Care Access Services (CCAC) is the first step. CCAC will assist you with your application to a Long-Term Care Home and can also provide you with information about alternatives to long-term care. Such alternatives could include supportive/assistive housing, in-home support services, retirement homes and seniors' apartments. The following steps take place before a new resident can be admitted into The Glebe Centre Long-Term Care Home:

  1. An assessment of the applicant must be carried out by  CCAC - who can be reached at 613-745-5525 or via their website
  2. A caseworker is assigned to guide the family through the process/rules and will determine what level of care is required and will also be the contact if an appropriate match becomes available
  3. The applicant's name, care needs and a list of preferred homes is added to the CCAC waiting list

In order to decide which homes are preferred, and the priority they should be in, families are encouraged to tour each home of interest. A tour of the Glebe Centre can be arranged through our receptionist (613-238-2727 ext 301) who can provide information on times, dates and availability of tours.

Both the resident and the Government contribute towards the total cost of accommodation in a long-term care facility. The resident pays accommodation charges only. The Government pays for the cost of nursing care and other services. The amount paid by residents for accommodation is called "co-payment". Rates are set annually by the Ministry of Health.
